ProjectPlusAuDHD: Thrice Exceptionality, What Exactly Is It?

The term thrice exceptionality (3e) is often debated because it can refer to two very different scenarios: on one hand, individuals with a neurodivergence, giftedness, and belonging to a minority group (LGBTQ+, ethnic minorities, physical disabilities); on the other, individuals presenting a neural architecture composed of ADHD comorbid with Autism and Giftedness.

In the first case, the problem with using this terminology is the tendency to label as "2e" (twice-exceptional) both those who have a single neurodivergence coupled with giftedness, and those who possess two neurodivergences plus giftedness. These situations, however, are profoundly different, especially in the diagnostic phase. In this article, and more broadly within this project, we will focus exclusively on the second scenario, also defined as the Gifted AuDHD profile.

The Differences

Twice-exceptional individuals often excel in specific domains, yet end up lacking in other areas. In the case of thrice exceptionality, the internal dynamic is far more complex. ADHD constantly seeks novelty and chaos, whereas Autism has a vital need for routine and order. This paradox makes the system almost "invisible" from the outside, enormously complicating clinical recognition. Add to this Giftedness, which acts as a powerful cognitive compensator, masking deficits and learning disabilities. The latter, despite being deeply felt by the individual, are consequently underestimated and left untreated.

The Diagnosis

As mentioned, diagnosing this specific comorbidity is extremely complex: the symptoms tend to mask each other or emerge only in specific contexts. This represents a critical issue, as it frequently leads to missed or, worse, misdiagnoses. The 3e profile, for example, is often confused with mood disorders or borderline personality disorder.
Furthermore, due to the excessive speed of thought and extraordinary pattern recognition capabilities, the individual can enter states of overexcitabilities (according to the studies of psychologist Kazimierz Dąbrowski) which are easily mistaken for clinical hypomania episodes, leading to a misdiagnosis of bipolar disorder. These states of overload are often exacerbated by sleep deprivation—due to mental hyperarousal—and by a profound loneliness that unites almost all individuals with thrice exceptionality.

Loneliness and Social Interactions

This chronic loneliness stems from the fact that the 3e's worldview is radically different from that of the majority. Their acute and all-encompassing sensitivity struggles to find resonance in neurotypical sensitivity, which the 3e individual sometimes perceives as superficial or incomplete.

Because of this divergent architecture of thought, 3e individuals may unintentionally hurt others by expressing concepts that are logical and normal to them, but which for the neurotypical listener result strange, out of place, or irritating. This tendency toward an extremely direct communication devoid of social filters causes the person to be erroneously perceived as "insensitive." This is the perfect representation of the Double Empathy problem (theorized by sociologist Damian Milton), according to which misunderstanding does not stem from an autistic deficit, but from a mutual incompatibility in languages and social expectations between neurotypicals and neurodivergents.

The Weight of Masking

The concept of masking is not exclusive to neurodivergence: most people adopt a "mask" to adapt to various social contexts. However, for individuals with double and thrice exceptionality, the cost of this operation is extremely high. The energy expenditure required to maintain the facade quickly leads to severe social burnout, a consequence already critical for those presenting Autism comorbid with Giftedness—a condition that leads to overanalyzing every single detail—but which in thrice exceptionality reaches its peak.

In the Gifted AuDHD profile, masking requires constantly managing two diametrically opposed neurobiological forces. On one hand, ADHD has a nature prone to sociality and stimulus-seeking, despite forcing the individual into exhausting self-control to mask impulsivity and appear "adequate." On the other hand, Autism demands order, predictability, and pushes toward withdrawal to avoid sensory overload. This paradox translates into fluctuating social masks that are disorienting: the 3e individual can brilliantly play the role of the "life of the party" one day, only to find themselves completely isolated and withdrawn the next day, with their social batteries at zero.

To survive this contrast, as they grow older, the 3e learns to leverage their giftedness to "engineer" their masking. Adaptation is no longer a simple instinctive imitation, but transforms into a protocol studied in minute detail and surgically calibrated for every specific occasion, in an exhausting attempt to balance the ADHD's need for interaction with the Autistic need for protection.
This mechanism, besides draining massive amounts of vital energy, can push the individual to desire total isolation for entire periods, making even a simple family interaction unbearable when the system's energy is directed elsewhere.

The Mission of ProjectPlusAuDHD

Living with thrice exceptionality means operating with a complex system that the outside world is simply not configured to read. You spend your life translating your internal state into a language that others can understand, burning yourself out in the process.

The goal of ProjectPlusAuDHD is to stop this energy leak. This project was born to create a safe space where individuals with thrice exceptionality can finally shut down their adaptation protocols. A place where there is no need to mask one's intensity, justify exhaustion, or explain one's speed, because you are in front of your "peers." Here, the goal is not to teach 3es how to adapt to the world, but to provide them with the tools to understand their own architecture and find, in mutual recognition, the right to exist without filters.
